All 3rd-5th graders are invited to join us at Hampton Presbyterian Church on Friday, November 1 from 6:00-10:00pm. $5.00 per child will be collected the night of the event to help cover costs.

I understand that my child will be engaging in supervised activities that may expose him/her to the possibility of injury. I give my permission for my child's participation in these activities except as otherwise specified in writing. I understand that every effort will be made to contact a parent/guardian in case of emergency. In the event of minor illness or injury, my child will receive basic First Aid. In case of serious illness or injury, I hereby give permission for medical care by a physician and/or hospital. (Please sign your name below.)
